The National Association of Counties (NACo) is hosting a webinar on Transitions of Care in Mental Health on Tuesday, June 18th, from 2:00 PM to 3:00 PM.
This is an educational presentation for mental health professionals and leaders looking for strategies to reduce hospital readmissions/criminal recidivism among people with serious mental illness in their communities. We will define a successful transition of care for adults with schizophrenia, review barriers to a successful transition, highlight specific examples of successful transitional care programs, explore how a multidisciplinary/collaborative approach can support successful transitions, and identify actionable strategies for potential implementation.
