MAX RFP BOARD: Partners for HOME Seeks Proposals for Prevention, Diversion, and Rapid Exit

From Partners for HOME:

Partners for HOME (PFH) is seeking proposals to launch an Atlanta CoC Prevention, Diversion, and Rapid Exit Program. Funds will be used to provide financial assistance, case management, transportation, equipment, and operation costs to assist 480 individuals/families in either preventing homelessness, diverting from homelessness, or to rapidly exit homelessness into a permanent housing solution. All three strategies are aimed to reduce the number of individuals/families entering the homeless system or to assist them in quickly exiting the system.

Prevention includes services for families and individuals at risk of imminent homelessness; Diversion includes services for individuals and families who are imminently homeless before they spend a night in shelter, unsheltered, or in a motel with a voucher in the City of Atlanta; and, Rapid

Exit includes services to exit individuals and families from shelter, transitional housing, or an unsheltered setting within indicated time frames.
